HOW TO GET CERTIFICATE OF ADMISSION

Description: Certificate is issued with utmost accuracy after discharge from the hospital.

Client/s: Discharged patients
Contact Person/s: Romarica Derodar (Medical Records)

Cebu City Medical Center (CCMC)
Availability: Monday-Friday / 8:00 AM - 5:00 PM
Duration of the Service: 20 minutes
Requirement/s: None
Fee/s: Medical Certificate Fee- Php 30.00

Procedure in Availing the Service:

Follow these steps...	Wait while Person-in-charge...	Approach...	It will take...
1. Proceed to Medical Records section.	Asks client of the date of discharge & cause of admission.	Cesar Cataluna	5 min.
2. Present valid ID or authorization letter, patient’s valid ID.	2.1. Assesses patient’s ID and authorization letter. 2.1. Retrieves patient’s chart. 2.3 Endorses patient’s chart to personnel making the medical record.		5 min.
3. Get charge slip.	Receives patient’s chart and issues charge slip.	Carleen Kimmy Bacalla	5 min.
4. Pay at the cashier.	Prepares the medical certificate.		5 min.
5. Present receipt & claim certificate.	Releases medical certificate & ask client to acknowledge receipt.
