HOW TO AVAIL OF CERTIFICATE OF INDIGENCY

Description: This service lets the client avail free and discounted services from other agencies through the Certificate of Indigency.

  Client/s: Indigent residents of Cebu City
  Contact Person/s: Social Workers

  Location: Department of Social Welfare Services (DSWS)
                       Katipunan, Labangon, Cebu City
  Availability: Monday-Friday / 8:00 AM - 5:00 PM
  Duration of the Service: 2 days, 1 hour and 30 minutes
  Requirement/s:
                                a) Barangay Certification / COMELEC Certification
                                b) BIR tax exemptions
                                c) City Assessor’s certification of no landholding

  Procedure in Availing the Service:

1. Submit requirements.	1.1. Interviews the client	District Social Worker	30 min.
	1.2. Conducts home visits and collateral information		1 day
	1.3. Prepares social Case Summary Report (CSR)		1 day
	1.4. Submits CSR to supervisor for signature		30 min.
2. Receive Certificate.	Issues certificate of indigency	District Social Workers/Dept. Head II	30 min.
